Assist Personal Activities

Assist Personal Activities is one of the services we offer to support participants with daily living activities, enabling them to live as independently as possible.  The scope of this service includes support with activities such as personal hygiene, grooming, dressing, meal preparation, eating, and toileting. It also includes assistance with mobility, such as transfers from one place to another, including in and out of bed, and moving around the home.

To ensure that our service meets the individual needs of the participant, we conduct an initial assessment to identify their strengths, preferences, and areas where they require support. This assessment allows us to create a tailored support plan that outlines the specific tasks and frequency of support required.

Our team works collaboratively with the participant and their support network, including family members, health professionals, and other service providers, to ensure the participant’s needs are fully met. We also regularly review the support plan to make adjustments as necessary, based on the participant’s changing needs and goals.
