Do I need an NDIS plan to use your services?
To use NDIS funding you need an approved NDIS plan. If you are still applying, contact us and we can point you in the right direction. You can also read our NDIS overview.
Can I change to Epitome Care from another provider?
Yes. NDIS participants can choose their own providers. We can work with you and your support coordinator to make the change as smooth as possible.
Do you support self-managed and plan-managed participants?
Yes. As a registered provider we can support participants whose plans are NDIA-managed, plan-managed or self-managed.
How do I make a referral?
